How to Register

We have an open list and welcome requests for registration from patients living in or moving to the practice area, please contact reception for more information.  Please note, we have a practice boundary and can only accept registrations from addresses within this boundary.

You can register by completing a form available from reception. To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery. Please bring details of previous GP and address.

You will be registered under a particular GP but you are free to see any of our GP’s for an appointment.

Please note we MUST have your NHS number in order to register you. If you do not know your NHS number, your previous GP practice will be able to give you this or you can use this finder via the NHS website

Form 3 – GMS 1

Please note that when you hand your registration form into reception you will be given a New Patient  Registration pack, which will need completing at this point.  In the pack you will receive an in-house questionnaire, consent forms for GDPR and a carers pack.

Temporary Resident

Form 4 – GMS 3

Please include your NHS number on the registration form, if  you do not know this you can find the information here.  You will also need to provide proof of identification and address when handing in your registration forms. You will need to provide one of the following documents from each of the lists:

Proof of Identity

Passport  /  Driving Licence  /   Birth Certificate  /  Marriage Certificate   /  Medical Card

Proof of Address

Utility Bill  /  Bank or Building Society Statement  /  Payslip (with address)  /  External Correspondence  (solicitors or estate agent showing name and address)

Patients will be registered to one of the doctors but are free to see any doctor for any problem. Obviously we prefer patients to stay with the same doctor for the same problem to maintain continuity of care.
